We promise that you will never feel "ordinary" with Dr. Keith, Dr.Chao and our expert staff. We go out of our way to ensure that you feel special with our comfortable setting and gentle touch dentistry.
We welcome new patients. We provide both general and specialty dental care.
Here are some helpful hints for your dental visit:
- Bring recent x-rays if possible or request them to be sent from your previous dentist. If xrays are unavailable, don't worry. We will take any necessary films using our digital xray system.
- Bring your dental insurance information to allow us to process dental claims more efficiently for you.
- Bring a list of any medications you are taking or are allergic to.
- Bring your physician's and pharmacy's name and phone number
- If you need antibiotics before your dental appointments, please take your prescription 1 hour prior to your appointment. This applies for patients with a history of a heart murmur, rheumatic fever, mitral valve prolapse, or recent joint/hip replacement. (If you are not sure if you need pre-medication, please contact your physician.)

If you would like to complete your paperwork before your visit, download our New Patient Forms and fill them out at your convenience. But don't forget to bring them with you!
Financial Information/Insurance:
To better serve you, we accept and file most insurances. This saves you from the hassle of dealing with the mountain of forms and paperwork that most insurance companies require. Uninsured? Visa, Mastercard, Discover and American Express are welcomed. Third party financing is also available.
Cancellation Policy:
Last minute cancellations deny other patients the benefit of treatment they need. We appreciate every effort you can make to give us as much notice as possible if you need to change an appointment. We do understand that occasionally an emergency will present itself and that there is no other option. Patients are charged $75 for these cancellations with less than 48 hours notice. If you have repeated occurrences we may suggest that you find another dental provider.
